UMNursing CCIC Model

The goal of the UMNursing Care Coordination Implementation Collaborative (CCIC) is to promote care coordination and patient-centered care across Maryland hospitals while continuing to expand the care coordination focus area/courses in academia.

Project background:

The 5-year project is supported by a Maryland Nurse Support Program II grant, which is funded through the Maryland Health Services Cost Review Commission and administered by the Maryland Higher Education Commission. It was built on the existing UMNursing program, a joint effort between the University of Maryland Medical System (UMMS) hospitals and the University of Maryland School of Nursing (UMSON).

Project goals include:

  • Establish a UMNursing CCIC infrastructure
  • Provide students and practicing nurses with actionable knowledge/skillsets in care coordination
  • Facilitate quality improvement projects in the areas of care coordination and patient-centered care
  • Assess the impact of the project on selected patient outcomes (e.g., patient experience with care, 30-day readmission, discharge planning), as well as nursing practice and education outcomes.

Major Project Accomplishments

RN-to-BSN Care Coordination with Health IT (CC/HIT) Focus Area

The Care Coordination with Health IT focus area allows RN-to-BSN students to explore how health information technology can facilitate the organization of patient care activities and information sharing among all patient care providers to achieve safer and more effective care.

The focus area includes three courses that all apply to the credits required for the RN-to-BSN program, meaning that completing this focus area doesn't require you to take any additional credits.

The focus of the interview is work done in a hub model in New Mexico. Quality improvement (QI) methods were introduced by the Institute Healthcare Improvement. The nursing home staff learned QI processes and began doing huddles. She discusses a cohort model used in this collaborative of nursing homes.  

In this video: Lerner discusses managing complex care patients during transitions from the hospital. She discusses three options: home with home health services, skilled care, and assisted living. The payment for these services is detailed including Medicare, Medicaid, and self-pay.

Karen Doyle, DNP, MBA, MS, RN, NEA-BC, FAAN
Senior Vice President of Patient Care Services and Chief Nursing Officer, University of Maryland Medical Center

About the expet: Doyle has served in a variety of executive roles in trauma and emergency nursing as well as women’s and surgical services over the last 35+ years. She currently serves as the senior vice president of nursing and operations at the University of Maryland Medical Center's R Adams Cowley Shock Trauma Center. Shock Trauma is the nation’s first integrated trauma hospital and is one of the largest trauma treatment and research centers in the U.S. In addition to her clinical and administrative responsibilities, Doyle serves as adjunct assistant professor at the University of Maryland School of Nursing.

Melissa Huber, BSN, MHA, CCM

About the expert: In addition to clinical nursing, Huber has 20 years of experience working in care management and program management across multiple health care settings. She is a board-certified case manager. Huber specializes in implementing population health strategies to deliver high quality and safe care while improving patient experience and reducing costs. Her philosophy of practice stresses that the patient is an equal partner in negotiating health services with their providers and strives to ensure their voice is represented until they are able to fully advocate for themselves. 


Nancy Lerner, DNP, RN, CDONA
Associate Professor and Program Director, Health Services Leadership and Management, University of Maryland School of Nursing

About the expert: Lerner has been an educator since 2008 following an extensive administrative career in all aspects of long-term care. Her primary teaching focus involves nursing administration, including theory application to health service management, complex adaptive systems, fundamentals of nursing and both management and DNP practicum experiences. Multiple publications and presentations center on quality in nursing home and assisted-living care, nursing assistant training, and job satisfaction in long-term care. Lerner has also served as an advisor and mentor to both DNP and management and administration students. She serves as a Pathway to Excellence appraiser and has served on multiple committees for nursing related groups.


Kristin L. Seidl, PhD, RN 
Clinical Data Scientist, Department of Quality Safety, University of Maryland Medical Center

About the expert: Seidl is a doctoral-prepared nurse with over 28 years of experience. In her PhD program, she focused on using genetic information for health care decisions. Since then, her professional practice has been directed to the field of health care quality and safety. Seidl holds a joint appointment with a large academic medical center and a nationally ranked school of nursing. Past leadership positions include director of nursing outcomes, research and evidenced-based practice and director of quality and patient safety officer. In her current role, Seidl is responsible for hospital quality data measurement and the management of 15 clinical registries. In her faculty role, she teaches doctoral level courses about outcome measurement, data analysis and implementation science. She is a member of the Quality and Safety Oversight Committee of a COVID field hospital.


Rebecca Tyrrell, RN-BC, CCCTM, RN
Care Manager, University of Maryland St. Joseph Medical Group Population Health, Maryland Primary Care Program

About the expert: Tyrrell is certified in American Nurses Credentialing Center, Care Coordination and Transition Management.  She has more than 20 years of experience in care coordination and transition care management. Prior to her current position, she was the transitional care coordinator at the University of Maryland Trauma Care Center until 2020. In her role, she developed a formal program to serve patients injured by trauma mechanism. The Trauma Transitional Care Coordination Program for the University of Maryland Shock Trauma Center, which she developed in 2014, remains in existence today.
