- Healthcare worker Intervention for Retention in HIV care and Viral Suppression (HIRHIVS)
- Description:
- The purpose of this study is to evaluate interventions done to improve HIV care outcomes. We will look at whether patients who received the more intensive intervention had better outcomes than patients who receive the less intensive intervention. The intensive intervention, Linkage to Care Plus (LTC+) was the use of nurse and community health workers (CHWs) to assist patients transitioning from the hospital to the home/community. Nurse and CHWs re-connected patients to outpatient medical care for their HIV. They met with patients while in hospital and provided extensive follow-up upon discharge from hospital to the community (including home visits, phone calls, accompanying to appointments). The less-intensive intervention (LTC) was conducted by navigators within the hospital. The navigators assessed and addressed barriers to care (such as housing, transportation, etc.) during the period of hospitalization or immediately after but without any direct contact with patients upon release from hospital.
