Between March and April 2023, the Health Suite providers completed 163 visits with 93 people, and 98% of neighbors who worked with a nurse to set a goal achieved that goal.
The CEC serves adults and children and offers a variety of programs that leverage University resources to meet community-identified needs such as exercise, food, and after-school and educational programming.
The CEC offers a free public health nurse program through its Health Suite that serves anyone in Baltimore city but focuses on residents in West Baltimore. Health Suite visits include interventions such as:
- health assessments
- screenings
- linkage to care
- care coordination
- clinical case management
- increasing neighbors’ knowledge of health conditions
- working with neighbors to create plans of care to manage their health
- supporting neighbors in between provider appointments
- assisting neighbors with telehealth appointments
- providing telehealth providers with real-time assessment findings
- providing health coaching and education
- connecting neighbors with resources to assist health and social needs (including case conferencing with other CEC departments)
- and reducing barriers to health living.
The Health Suite is an interprofessional clinical site for UMSON and other UMB students, led by Kelly Doran, PhD ’11, MS ’08, RN, FAAN, associate professor. About eight nursing students, including Bachelor of Science in Nursing (BSN), RN-to-BSN, and Doctor of Nursing Practice students, participate in a clinical experience there one day a week. Health Suite appointments can be in person at the CEC, over the phone, at a provider’s office, or at a housing site.
The CEC Health Suite is located at 16 S. Poppleton St., Room 304D, in Baltimore. It also serves as a partner in the West Baltimore RICH Collaborative.
