If you follow the link to the UM Environmental Health and Safety (EHS) Web page you will find the online forms needed to get proof of coverage and claims history.
In order to provide this information, EHS must also obtain confirmation of enrollment from the Office of the Registrar.
To allow enrollment confirmation to be released to EHS, graduates and former students must also complete an online request form from the Office of the Registrar.
Completing this two-step process online should save some time, but the process still takes at least 3 weeks.
Please be advised that your request cannot be processed until you graduate. Requests made before your graduation date will be held until such time.
