  • Request one Blackboard Web-enhanced course site for each course
• Assessment Form
  • Create an online assessment 
  • Complete assessment form, save to your desktop as a PDF file and return with assessment to: Leah Smith, lead data control clerk 
  • Note: The Office of Learning Technology requires that the assessment and form be submitted at least five business days prior to the assessment’s opening date.
• Organizations/Community Request Form
  • The Organizations/Communities section of Balckboard is for organization/group sites. These sites can be used for collaboration, communication, and online organization of materials.
